How to Book
There are two convenient ways to book your initial assessment with Pearson Health. Please contact us any time to discuss any of these options further. We’d be happy to hear from you!
Once your referral form has been reviewed by our practitioners, we will contact you to book an appointment.

With Referral
To provide you with the best quality care, we require a referral from your primary care provider (GP/NP). This gives our practitioners access to your complete medical history, so we can provide the best advice possible. It also ensures continuity of care with your primary care team. Please ask them to complete the form below or use their standard referral form & fax to 1-844-240-9273.

Self-Referral
For Ontario residents without a primary care provider, self-referrals may be possible for certain qualifying conditions. A one-time, non-refundable, $65 self-referral fee will also apply due to the additional work involved which is not covered by OHIP. This is due at time of booking. Please complete the Self-Referral Screening Form below to get started.
Cost
At Pearson Health, we pride ourselves on providing the most comprehensive & personalized care available, to support you in every stage of your care. All physician / nurse practitioner visits are covered under OHIP for Ontario residents with a valid health card.
For patients booking an appointment with a referral from their primary care provider, there are no additional fees.
For patients who are accepted under our self-referral program, a one-time, non-refundable $65 fee will be applied at the time of booking, due to the additional administrative work required by our patient care team.
Medical notes, insurance claims forms and other items not covered by OHIP are also charged on an as needed basis. Rates will be provided, upon request.
